The report includes an extract from the incident report and a copy of the victim statement provided to the WA Police Force.

This information is designed to assist you (or your representative) to assess the circumstances of the incident before completing an application for CIC.
The report is available for any incident investigated by the WA Police Force in which you suffered an injury (bodily harm, mental and nervous shock, or pregnancy).
Police investigations and court proceedings must be finalised before information will be released.

Note: Where documentary proof of representation cannot be provided due to cultural practices or family arrangements, a letter of consent may be accepted by the victim / witness (if aged 15 or older) authorising the release to the representative. Such applications must be supported by Photo ID or other proof of identity of the victim / witness. Alternatively, Public Access may attempt to verify the legitimacy of the application using the information holdings of the WA Police Force. |
